Understanding your support options

The language around services, waivers, authorizations, and provider availability can feel complicated. This overview explains the options Life Choice currently publishes and where to begin.

CIH

Community Integration and Habilitation Waiver

The CIH Waiver supports eligible individuals with intellectual and developmental disabilities through authorized home and community-based services. The approved person-centered plan identifies the services and outcomes authorized for the individual.

FS

Family Supports Waiver

The FS Waiver supports eligible individuals with intellectual and developmental disabilities in home, family, and community settings. The approved plan and authorization determine which services are available to the person.

Private pay

Private pay

Private-pay services may be an option when a family wants to purchase an available service outside of, or in addition to, an authorized waiver arrangement. Life Choice must confirm that the requested service can be provided privately, along with the scope, schedule, rate, agreement, and availability.

Life Choice can review the service request, location, schedule, and support needs for provider fit.

Outside our control

Life Choice cannot guarantee waiver eligibility, authorization, placement, funding, or start dates. The applicable Indiana agencies, planning process, and approved person-centered plan determine eligibility and authorization.

If you already have a case manager, include them in the conversation about authorized services and provider choice. If you are new to the process, use the current official Indiana resources and contact Life Choice with general questions about its own services.

Start with the service you are seeking, the person’s city and county, and the best way to reach you. A member of the Life Choice team will review the inquiry and follow up about provider fit and next steps.

Please do not include diagnoses, medications, detailed behavior information, risk details, Social Security numbers, or Medicaid numbers in your first message. This form is not monitored for emergencies. If there is an immediate threat to life or safety, contact the appropriate emergency service.
