About Life Choice

Choice, dignity, and support for everyday life

Life Choice exists to help people live with more voice, connection, and opportunity. We provide person-centered support shaped around each individual’s goals, routines, relationships, strengths, and approved plan.

Who we are

Life Choice is an Indiana provider of home and community-based services for individuals with intellectual and developmental disabilities. We partner with individuals, families, guardians, case managers, and other members of the support team to understand what matters most and translate that understanding into practical support.
 

No two people define a good life in exactly the same way. For one person, progress may mean building confidence with a daily routine. For another, it may mean participating more fully in the community, strengthening communication, or giving a family caregiver time to rest. Our job is to listen, coordinate, and support the goals in front of us.

Support is connected to daily routines, responsibilities, interests, health and safety needs, and approved goals.

We focus on abilities, communication, learning, and what helps each person participate more fully.

Coordinate clearly

We work with the support team so roles, expectations, and next steps are easier to understand.

Adjust thoughtfully

Needs can change. Support should remain responsive within the person’s approved plan.

Start with the service you are seeking, the person’s city and county, and the best way to reach you. A member of the Life Choice team will review the inquiry and follow up about provider fit and next steps.

Please do not include diagnoses, medications, detailed behavior information, risk details, Social Security numbers, or Medicaid numbers in your first message. This form is not monitored for emergencies. If there is an immediate threat to life or safety, contact the appropriate emergency service.
