Residential Habilitation and Support
Individualized support can help a person manage everyday routines, build skills, stay connected, and participate more fully at home and in the community. Life Choice provides residential habilitation and support according to the person’s approved plan and authorized needs.

Support for everyday life
Residential support is not one-size-fits-all. The team begins with the person’s goals, abilities, preferences, routines, communication, relationships, and health and safety plan. Support is then organized around the activities and outcomes identified in the approved person-centered plan.
Support may include
Personal routines and activities of daily living
Meal planning, shopping, preparation, and household tasks
Errands, appointments, and participation in community life
Practice with communication, decision-making, and daily living skills
Support for leisure, relationships, and personal interests
Implementation of approved health, safety, risk, and behavior strategies
Coordination with family, guardians, case managers, and other support-team members
Built around the approved plan
The exact tasks, schedule, setting, and level of support vary by person. Services must follow the approved plan, authorization, applicable waiver requirements, and Life Choice’s current provider availability.
Program note
Residential Habilitation and Support is generally associated with the Community Integration and Habilitation Waiver. Eligibility and authorization are determined through the applicable Indiana process.

Is residential support the same for every living arrangement?
No. The authorized service and support tasks depend on the person’s living arrangement, assessed needs, approved plan, and applicable program requirements.
Can families or case managers ask about availability?
Yes. Individuals, families, guardians, and case managers can contact Life Choice. Availability must be confirmed for the specific service, location, authorization, and staffing needs.
