A meaningful day looks different for every person. Day habilitation can support learning, practice, relationships, and participation through activities connected to the person’s goals and community.

Skills that support everyday life
Life Choice organizes day support around the outcomes in the person’s approved plan. Activities should have a clear purpose and create opportunities to practice skills in ways that are useful, respectful, and connected to real life.

Self-care and daily living skills
Communication and social interaction
Community living and navigation skills
Sensory and motor development
Making choices and expressing preferences
Building relationships and natural supports
Participating in activities connected to personal interests and goals
Community is more than a destination
Community participation should create genuine opportunities for belonging, contribution, learning, and relationships. The activity, setting, level of assistance, and pace should reflect the person rather than a generic schedule.
Individual planning
The approved plan determines the intended outcomes and authorized support. Life Choice will confirm whether the requested service, location, schedule, and staffing needs are currently available.
