Life Choice works with the person and their support team to review the requested service, routines, goals, location, schedule, communication, and level of assistance before confirming fit and availability.

Not every service is available through every program or in every location. The approved plan, current certification, authorization, and provider availability must align.

Start with the service you are seeking, the person’s city and county, and the best way to reach you. A member of the Life Choice team will review the inquiry and follow up about provider fit and next steps.
